Signs Your Lungs Are Not Working Properly: 10 Warning Signs to Know

📖 9 min read🗓 June 2026✓ Medically reviewed

Recognising the signs your lungs are not working properly is one of the most important things you can do for your long-term respiratory health. Lung conditions — including COPD, lung cancer, and pulmonary fibrosis — have far better outcomes when caught early. Yet many people dismiss early warning signs as "just getting older," delaying diagnosis by years.

Emergency symptoms — act immediately

Sudden severe breathlessness, chest pain, coughing blood, bluish lips or fingertips, or confusion with difficulty breathing are emergency symptoms. Call 999 or 911 immediately — do not drive yourself to hospital.

⚠️

1. Breathlessness Disproportionate to Exertion

Getting winded climbing one flight of stairs, becoming breathless during a slow walk, or finding activities that were easy last year now leave you gasping — this pattern of exertional dyspnoea is one of the earliest and most consistent signs of COPD, heart failure, and other significant conditions. A simple spirometry test (15 minutes at your GP surgery) can measure your lung function precisely.

2. Persistent Cough Lasting More Than 8 Weeks

A chronic cough is one of the most important signs your lungs are not working properly. While many chronic coughs have benign causes, persistent cough is a presenting symptom in up to 65% of lung cancer cases at diagnosis. See our guide on natural remedies for chronic cough, but get it evaluated first.

3. Coughing Up Blood or Blood-Streaked Mucus

Haemoptysis always requires urgent medical review regardless of quantity. Do not wait to see if it resolves — see your GP or go to urgent care the same day.

4. Chronic Wheeze

A persistent or recurrent wheeze, or a new wheeze developing in adulthood, is a clear sign your lungs are not working properly and requires assessment for asthma, COPD, or other obstructive airway conditions.

5. Frequent Respiratory Infections

Getting more than 2–3 chest infections per year, or finding they take unusually long to resolve, is a sign your lungs' natural defence mechanisms are compromised. When mucociliary clearance and local immune defences are impaired — by COPD, bronchiectasis, or immune dysfunction — infections become more frequent and severe.

6. Fingernail Clubbing

Clubbing — a widening and rounding of the fingertip with loss of the normal nail angle — is a physical sign associated with chronic low oxygen levels. It's associated with serious conditions including lung cancer, pulmonary fibrosis, and bronchiectasis, and develops gradually so many people live with it unknowingly.

7. Blue Tinge to Lips or Fingertips (Cyanosis)

A blue or grey discolouration of the lips, tongue, or fingertips indicates significantly reduced oxygen saturation in the blood. Central cyanosis (blue tongue and lips at normal temperature) requires urgent medical evaluation. A home pulse oximeter can give you an immediate oxygen saturation reading — below 95% at rest warrants attention; below 92% is urgent.

8. Reduced Exercise Tolerance Over Time

Progressive reduction in exercise capacity over 6–12 months is a subtle but significant sign your lungs are not working as well as they should. COPD in particular causes gradual, often imperceptible decline in lung function over years. The key question: is your tolerance declining year on year?

9. Unexplained Weight Loss With Respiratory Symptoms

Significant unintentional weight loss (more than 5% of body weight over 6 months) combined with any respiratory symptoms is a potential red flag for lung malignancy. This combination should prompt urgent GP evaluation.

10. Morning Breathlessness or Nighttime Symptoms

Regularly waking up short of breath or experiencing breathing difficulties that disturb sleep are signs your lungs or airways are not working properly. This pattern is particularly characteristic of asthma, sleep apnoea, and nocturnal GORD. See: why am I short of breath when I wake up?

What to Do If You Recognise These Signs

  1. See your GP. Don't delay — early diagnosis consistently improves outcomes across all respiratory conditions.
  2. Keep a symptom diary for 1–2 weeks before your appointment — timing, severity, triggers, and what helps.
  3. Avoid smoking or passive smoke exposure from today.
  4. Begin breathing exercises and review your indoor air quality while awaiting your appointment.
